During each subsequent year that the plan is renewed, each Member receives the same benefits (subject to any changes to the plan made effective prior to renewal).
What Are The Exclusions
This is NOT dental insurance. It is a dental discount plan that can only be used at this office.
This plan in not applicable to dental services paid by a third party, medical, workers compensation, dental insurance, disability, auto or otherwise.
To receive the 20% discount, payments must be made at the time services are provided. If full payment is not made at the time services are provided, such services will be charged at the usual, non-discounted, rate.

Discounts under your membership do not apply to any treatment prior to joining.
Annual Membership Fee
Any person can be a member, for example, you can buy a membership for yourself or any other individual not having dental insurance. Memberships may be purchased for one person or for multiple members of a family. “Family” includes spouses and dependent children under the age of 21.
The plan year starts on the date that the application is completed and the appropriate fee is paid. The plan can be renewed on the same date of each year.
Annual Membership Rates
Cancellation
During the first 10 days of the plan year, a member may cancel his or her participation in this plan by providing written notice of cancellation, and paying in full the amount of all rendered discounts received.
The 12-month membership fee is due in full upon joining.
All applications for a family must be submitted at one time to receive the Membership discounts. Any additional family members can not be added to the plan after that date until the renewal date.
Membership is effective the date payment is received.
Payments are due when services are rendered. For services that require multiple visits, the full amount can be divided among the required visits but must be paid in full on the date treatment is completed. If payment is not received in full for service on date completed the regular fee will be charged for the service.
It is the sole responsibility of the member to maximize their benefits by arranging all appropriate appointments within the 12 month membership period. If the appointments are not used, the member will not be entitled to a refund or an extension of the current plan without renewal payment. The anniversary month of your membership is the month that you make payment for the discount plan.
